根据《精神疾病诊断与统计手册第5 版》 (DSM-5),孤 独 症 谱 系 障 碍(autism spectrum disorder,ASD)是在社会性交往方面存在持续性的 缺陷,有局限重复的行为、兴趣以及活动 ,上述症 状常出现于童年早期(但也可能由于个体的社会 性需求尚未达到权限水平,而使症状无法全部表 现),同时存在日常功能受限或损伤。流行病学 研究发现全球 ASD 患病率逐年快速增长。在美国, ASD 患病率为 11.3/1 000[1]。在中国,ASD 患病率 为 26.6/10 000,其中中国大陆典型孤独症患病率为 11.8/10 000[2]。
随着对 ASD 的研究,近年来,有关 ASD 特质 在非 ASD 患者中的研究越来越多,日益得到学者 们的重视,并取得了一系列的研究进展,现就有 关的文献综述如下。 1 ASD 特质
1943 年 Kanner[3] 首次提出,一些ASD患儿 的父母不懂如何与他人建立社交关系,在建立社 交关系上存在明显的困难。Folstein 等[4]1977 年对 ASD 双生子的研究显示,未达到 ASD 诊断的同胞 中存在较轻的特质,而且这些特质在质上与 ASD 所表现的症状相似。Bolton 等[5] 于 1994 年对 ASD 家系研究中定义了孤独症广泛表型,即社交互动 障碍、言语交流障碍或刻板行为。1995 年 Bailey 等[6] 对 27 对 ASD 双生子的研究显示,60% 同卵 双生子均患有 ASD,92% 的同卵双生子均表现与 社交相关的 ASD 特质。
在 ASD 患者的一级或二级直系亲属中常有非 精神病理学的亚临床的孤独症特质,称为孤独症 广泛表型(broader autism phenotype,BAP)[7] 。BAP通常在孤独症特质相关量表筛查分数高于划界分, 但是不满足 DSM-IV 对 ASD 的诊断标准,同时在 基因、认知、行为方式等方面又有特征性表现。 研究发现与正常发育儿童的父母比较,ASD 患者 的父母有固执、冷漠和焦虑的人格特征,同时存 在人际交往困难,语用语言能力差,也有重复刻 板行为[8, 9]。Constantino 等[10] 对 178 名广泛发育障 碍(pervasive development disorder,PDD) 儿 童 与 48 名正常发育儿童研究发现,社交互动障碍是孤 独症特质的核心表现。Silverman 等[11] 对 ASD 家 系研究发现,未达到 ASD 诊断标准的同胞也存在 不同程度的言语交流障碍和重复刻板行为。Spiker 等[12] 研究发现,ASD 同胞的重复刻板行为与其家 系中 ASD 患者的疾病严重程度呈梯度相关。
ASD特质不仅存在于ASD患者及其家属 (BAP),也存在于一般人群中。ASD 特质主要 包括社交相关特质和非社交相关特质,社交相关 特质主要指社交互动障碍和言语交流障碍,非社 交相关特质主要指重复刻板行为和兴趣范围狭窄。 ASD 特质在人群中的分布特征是独立且连续的, 又是可遗传的[13, 14]。ASD 特质也受到基因和环境因 素共同作用。
研究发现,ASD 的遗传度在 90% 以上[15]。在 单卵双生子中,若其中之一诊断为 ASD,则另一 个 60%~90% 患有 ASD;在异卵双生子中若其中之 一诊断为 ASD,则另一个 30%~60% 患有 ASD。在 ASD 父母中,约 33% 父亲和 23% 母亲存在不同程 度的孤独症特质[16] 。最早研究数据显示,在 ASD 先证者的同胞中,约 20% 有一种或多种 ASD 的核 心症状(社交互动障碍、言语交流障碍和重复刻 板行为)[17]。但 De la Marche 等[18] 研究发现,孤 独症特质仅在 ASD 父亲较明显,在其母亲及同胞 不明显。
在一般人群中,孤独症特质的遗传度是中度 到高度,又有性别差异,孤独症特质在女性的遗 传度为 53%,在男性遗传度为 72% [19],也有研究 发现父母存在社交互动障碍与其子女社交互动障 碍的相关性为 0.40[20]。Constantino 等[21] 对 7~15 岁 发育正常的双生子研究发现,孤独症特质(主要 指社交互动障碍)在男性中检出率为 1.4%,在女 性中检出率为 0.3%。 2 筛查孤独症特质的常用量表 2.1 社交反应量表
社 交 反 应 量 表(social responsiveness scale, SRS)由 Constantino 等[22] 于 2005 年编制,适用于 4~18 岁儿童,由儿童的主要抚养者根据其近 6 个 月在自然社交环境中与他人的社交表现来填写。 该量表有 65 个条目,包括 5 个亚量表,分别为 社交知觉(social awareness)、社交认知(social cognition)、 社 交 沟 通(social communication)、 社交动机(social motivation)及孤独症行为方式 (autistic mannerisms)。每个条目有 4 级评分:l 代表“没有”,2 代表“有时”,3 代表“经常”, 4 代表“总是”。SRS 总分为 0~195 分。量表分数 越高,社交障碍程度越严重;分数越低,社交能 力越好。在国内,陈秋如等[23] 对 SRS 信效度检验 显示总分 >60 分考虑孤独症可能性较大,SRS 总 量表的内部一致性为 0.95,重测信度为 0.96,各 亚量表信效度均较好。
SRS 旨在对孤独症症状和特质进行量化的评 估,可用于区分 ASD 和其他精神疾病。SRS 分数 在人群中是连续分布的,在不同评估者间有较高 的一致性,受被试者智力因素影响小[24]。该量表 已在不同国家和地区得到应用,临床和非临床研 究信效度均较好,且多用于孤独症家系和孤独症 特质研究[25]。在台湾,该量表跨文化研究显示有 良好的信效度[26],同时研究发现,在儿童青少年 中孤独症特质的社交损害加重了其社会和学校适 应不良[27]。研究发现,在青少年中,有孤独症特 质的人群其 SRS 分数与其右侧颞上回皮层的变薄 有关[28]。也有研究发现,在一般人群中,SRS 量 表筛查有孤独症特质的人群,其脑电图 delta 波和 theta 波显示的大脑功能连接减弱与 SRS 分数呈正 相关,在一般人群中孤独症特质影响其大脑的功 能连接[29]。 2.2 孤独症严重程度量表
孤 独 症 严 重 程 度 量 表(autism spectrum quotient,AQ)由Baron-Cohen 等编制[30, 31],儿童版(适 用于 4~11 岁儿童)和青少年版(适用于 12~16 岁) 均由 ASD 主要抚养者根据其实际情况进行评估。 AQ 有 50 个条目,总分 0~150 分,每个条目有 4 个选项:“完全同意”“少许同意”“少许不同意”和“完全不同意”,5 个亚量表分别为社交技巧(social skill)、注意转移(attention switching)、细节关 注(attention to detail)、言语交流(communication)、 想象力(imagination)。该量表分数越高,表明孤 独症特质越明显。该量表已在国外广泛应用,其 中在亚洲的日本、中国台湾和马来西亚已有该量 表信效度检验,均证实有良好信效度[32]。在台湾 AQ 应用中,在结构因子分析中根据调查的实际情 况调整 AQ 为 35 个条目,且测量维度不变,信效 度检验显示内部一致性为 0.54~0.88[33]。
AQ 不仅可以筛查 ASD(尤其高功能孤独症), 也可以在智力正常人群中筛查出孤独症特质。研 究显示,AQ 分数可预测部分社会认知功能。在 ASD 中,AQ 分数越高,心理理论测验分数越低[34]。 在一般人群中,AQ 分数与社交技巧呈负相关,与 其细节关注能力呈正相关[35]。有研究发现,在一 般人群中,感觉处理问题与其孤独症特质呈正相 关[36]。也有研究发现,在 AQ 分数较高的人群在 识别愤怒、厌恶和悲伤的情绪准确度较低,且需 要较高的情绪化内容[37]。AQ 分数高的受试者在适 当区分社会行为和对社会行为反应的理解及认识 上较差,同时存在人际关系较差[38]。 2.3 孤独症广泛表型问卷
孤 独 症 广 泛 表 型 问 卷(broader autism phenotype questionnaire,BAPQ) 由 Hurley 等[39] 于 2007 年编制,评估成人孤独症特质及言语特征, 包括被试者的社交困难、语用困难和固执刻板个 性特征,主要用于 ASD 表型研究。BAPQ 有 36 个 条目组成,3 个亚量表:冷漠(aloof)、刻板(rigid) 和实用语言(pragmatic language),每个条目 6 级 评分:1 代表“很少”、2 代表“少”、3 代表“偶 尔”、4 代表“时常”、5 代表“经常”、6 代表“总 是”。BAPQ 分数越高,表明孤独症表型越明显。 该量表的灵敏度及特异性均在 80% 以上,总量表 及各亚量表的内部一致性均在 0.8 以上。该量表在 ASD 家系和一般人群均有应用,研究显示对于评 估孤独症特质 BAPQ 与 SRS(成人版)有较好的一 致性[40]。
采用BAPQ 由ASD 父母对夫妻双方相互评 测,结果显示 ASD 父亲冷漠亚量表分数较高,性 格冷漠,ASD 母亲刻板亚量表分数较高,行为刻 板,孤独症广泛表型存在性别差异[41]。在大型社 区人群中,采用 BAPQ 调查,结果显示:ASD 父 母 BAP 的检出率为 14%~23%,正常发育儿童父母 BAP 的检查率为 5%~9%[42]。 3 孤独症特质神经影像学研究 3.1 ASD 亲属相关研究
正 电 子 发 射 断 层 摄 影 术(positron emission tomography,PET)研究显示,ASD 患者的父母在右 顶叶和双侧额下叶脑血流灌注减少[43]。也有研究 显示,ASD 患者及其父母存在大脑皮层 5-HT2 结 合电位减低,且大脑皮层 5-HT2 结合电位与血小 板5-HT2 水平呈负相关[44]。结构 MRI 研究显示, ASD 患者及其父母的海马体积大于正常对照,ASD 患者左侧杏仁核体积小于其父母和正常对照[45]。 ASD 父母海马体积的增大可能是 ASD 潜在遗传学 基础的神经影像学表现。同时有研究发现,与正 常对照比较,ASD 患者的父母在左侧小脑半球前 部灰质体积减少,在左侧小脑半球后(包括梭状回、 楔叶)、右侧枕中会和颞中回灰质体积增加[46]。 但 Palmen 等[47] 研究发现,ASD 的父母双方与对照 组夫妇大脑体积无明显差异。功能 MRI 研究发现, Asperger 综合征患者的父母,在视觉搜索任务双侧 视皮层激活减弱,在情绪识别任务中左侧颞中回 和左侧背外侧前额叶激活减弱[48]。在语音加工任 务中,ASD 患者的父母右侧颞上回、左额下回和 左岛叶激活异常,语音处理缺陷可能是 BAP 的核 心特质[49]。
PET 研究显示,ASD 患者的同胞在右额叶、 右尾状核和左顶叶脑血流灌注减少[43]。在 MRI 扩 散张量成像(diffusion tensor imaging,DTI)研究中, ASD 患者及其同胞在前额叶、顶叶和颞叶白质各 向异性值(diffusion anisotropy,FA)减小[50]。功能 MRI 研究显示,在图片镶嵌任务中,ASD 患者及 其同胞在左侧颞中回前部、额下回和颞中回前部 激活增强[51]。在面部表情任务中,ASD 患者及其 同胞在与同情心和表情加工相关的脑区(左侧额 上回、双侧颞极、左侧背外侧前扣带回和梭状回等) 激活减弱[52]。ASD 同胞孤独症特质的表现与其神 经影像学的改变相一致。 3.2 一般人群中孤独症特质 MRI 研究
在一般人群(非 ASD 患者及其家系)中,静 息态 MRI 研究显示,SRS 分数与前扣带回膝前部 与岛叶前中部之间的功能接连呈负相关。低水平 的孤独症特质与岛叶前中部和扣带回膝前部功能 连接呈正相关[53]。同时,Jakab 等[54] 研究发现在 静息态 MRI,孤独症特质的水平与右后侧扣带回 的激活呈负相关。近年来,在一般人群中 AQ 的筛 查与大脑磁共振相结合的研究较多。研究者发现, 与 AQ 得分较低者相比,AQ 得分高者颞上回后部 (posterior superior temporal sulcus,pSTS)白质减少, 在 stroop 任务中 pSTS 激活减弱。pSTS 与处理社会 相关刺激有关[55]。同时,结构 MRI 显示高水平的 孤独症特质与右侧 pSTS 皮层变薄有关[56]。有研究 发现,AQ 总分与颞上回 - 杏仁核白质连接体积呈 正相关,颞上回与杏仁核白质连接主要负责面部 表情处理[57]。也有研究发现,有孤独症特质者在 双侧颞叶梭状回和海马回 FA 值减少[53]。
在一般人群中,高水平孤独症特质者与 ASD 患者的大脑结构改变是相似的,主要涉及社会信 息加工和表情处理的脑区。这进一步说明了社交 互动障碍是孤独症特质的核心症状。 4 孤独症特质相关认知研究
在执行功能的研究中,结果显示 ASD 的父母 及其同胞存在执行功能受损,尤其注意灵活性和 计划[58, 59]。在韦氏智力测验中,ASD 的父母及其 同胞在言语智商、拼写和阅读得分数较低[60]。在 眼神测试中,BAPQ“冷漠”分数高的 ASD 父母不 能准确地从眼神中认识到所表达的情绪,存在社 会认知缺陷[61]。在面部表情识别中,BAPQ“冷漠” 分数高的 ASD 父母对于眼部关注减少,更多关注 对方的口部,有着特殊的面部表情处理方式[62]。 有研究发现,孤独症的父母存在言语工作记忆障 碍,在心理推理任务中表现较差[63]。也有研究发 现,ASD 及其有孤独症特质的父母均存在社会认 知缺损,社会认知的神经环路与正常人存在差异。 大多数研究者认为社会认知可能是孤独症从认知 过程到大脑结构的内表现[64]。 5 总结
Skuse 等[65] 基于学校的调查研究显示,在小 学拒绝接纳的学生中,约三分之一有轻度到中度 的类似孤独症行为(ASD 特质),约 70% 有 ASD 特质的社会认知功能障碍。Skuse 等[66] 研究也发 现在小学生中,ASD 特质增加了其社会和行为问 题。对 ASD 特质的认识和研究可以对学校拒绝接 纳的小学生进行适当的干预与指导,减少其退学 率。
此外,孤独症特质处于疾病和遗传基因之间, 与遗传基因联系密切,同时受到外界环境的影响 较小,因此通过孤独症特质的研究对 ASD 复杂遗 传机制的探索有着独特的优势。目前,国内对孤 独症特质及孤独症广泛表型的研究较少,存在的 问题主要有:缺少有效的筛查评估工具,在 ASD 亲属及一般人群中孤独症特质的检出率及遗传度 尚不明确,ASD 及其亲属认知功能及神经影像学 的研究较少。同时,在国内外均缺乏对 ASD 患者 及孤独症特质人群的基因与神经影像学的综合研 究。
考虑到目前国内研究的不足,我们需要引进 国外相关的适合跨文化研究的量表,结合国内实 际情况,完成量表的信效度检验。使用信效度较 好的量表,筛查出 ASD 亲属及一般人群的孤独症 特质,也可进一步探索对于一个与多个 ASD 先证 者家系的遗传机制是否存在差异。同时,应把遗 传学研究与神经影像学有效结合,建立微观与宏 观的连接。通过对 ASD 表型及特质的研究,希望 可以对认识 ASD 的精神病理学及遗传机制进一步 阐释,分辨影响 ASD 易感性的生物学途径,同时 为 ASD 干预提供有力的证据。
