骨髓增生异常综合征(myelodysplastic syndromes,MDS)是一组克隆性疾病,以无效造血、 不同程度的单系或多系血细胞减少、较高风险向 急性髓系白血病(acute myeloid leukemia,AML)转 化为特点,临床异质性强。儿童MDS 是一种少见病, 在儿科肿瘤中所占比例不足5%[ 1, 2, 3 ]。由于发病率低, 现有研究对儿童MDS 的疾病本质了解较少。儿童 MDS 与成人MDS 相比,主要治疗目标是治愈而非 缓解和维持,因此对儿童MDS 进行分子生物学水 平的研究、探索疾病本质,具有更为重要的意义。 本文对近年来国外关于儿童MDS 的分子生物学方 面的研究现状作一简单阐述。 1 儿童MDS 特点
儿童MDS 与成人MDS 在流行病学、生物学、 细胞遗传学等方面都有很大的不同[ 4 ]。儿童MDS 每年发病率大约在0.5~4/106,与成人每年发病率 20~40/106 相比较低[ 5, 6 ]。在生物学和临床表现上均 不相同:(1) 成人中的亚型难治性贫血伴环形铁 粒幼细胞增多(RARS)和5 号染色体长臂缺失(5q-) 在儿童中罕见[ 1 ]。(2) 成人MDS 中难治性贫血 (RA)亚型临床表现多见贫血,而儿童病例中性 粒细胞减少和血小板减少相对更为常见。因此, 难治性血细胞减少(refractory cytopenia,RC) 更适 合用来描述不伴有原始细胞增多的儿童MDS[ 7 ]。 (3) 与成人RA 相比,大多数儿童RC 患者骨髓 增生减低,而非增生活跃。骨髓细胞数减少的发 生率可超过70%[ 8 ]。(4)儿童MDS 约有30% 可 检测到7 号染色体异常(包括单倍体7),是最常 见的细胞遗传学异常[ 9, 10 ]。因此,许多人认为儿童 与成人MDS 可能是性质不同的两类疾病,现有对 成人MDS 的发病机制研究结论,不能套用于儿童。 2 髓系肿瘤常见的基因突变
Kelly 和Gilliland 假定髓系肿瘤的发生至少需 要两类突变:II 类突变导致白血病细胞的分化受阻, I 类突变则在白血病细胞的过度增殖中发挥作用, 两类突变之间的非随机协同作用在髓系肿瘤中得 到了广泛研究[ 11 ]。现在普遍认为,成人MDS 的 发病也是一个由两类突变相互作用参与的多步骤 过程,这一点与AML 相似。但这两类异常在儿童 MDS 中参与作用的信息还很缺乏。有人研究了一 组107 例儿童MDS 跨国队列,其中7.5% 的患儿 发现了I 类突变,包括FLT3-ITD、 NRAS、KRAS 以及PTPN11;16% 的患儿检测到II 类突变,包 括MLL、RUNX1 重排、RUNX1 突变、CEBPA、 NPM1 和 NUP98[ 12 ]。这项研究表明,儿童MDS 患 者中I 类突变与II 类突变的发生率较成人患者较 低,尤其是调节组蛋白修饰和DNA 甲基化的基因, 这一点也表明年龄老化影响了这些基因的突变状 态。
核仁磷酸化蛋白(nucleophosmin,NPM) 是 一类穿梭于核仁、核质和胞质的蛋白质,发挥调 节造血的功能。NPM1 基因负责编码NPM,高表 达于增殖活跃的细胞包括肿瘤细胞以及干细胞, 在肿瘤形成过程中具有重要的作用。NPM1+/- 杂合 突变的小鼠可以进展为伴有人类MDS 特征的血液 病[ 13 ]。NPM1 基因第12 外显子的突变是目前成人 AML 中检出率较高的一种分子异常,在核型正常 的AML 中发生率高达40%~60%。有研究者检测 了17 例MDS 患儿,只有1 例患儿发现了此突变[ 14 ]。
酪氨酸蛋白磷酸酶非受体型11(tyrosineprotein phosphatase non-receptor type 11,PTPN11) 基因位于12q24.13,编码含Src 同源区2 的蛋白酪 氨酸磷酸酶(SHP2),SHP2 通常发挥增强信号的 作用,将活化生长因子和细胞因子受体发出的信 号转送到RAS 和其他胞内信号分子,SHP2 功能 丧失对造血系统的进展有重要的影响[ 15 ]。近年来 国内外检测到PTPN11 基因的突变可以发生在许 多髓系肿瘤,特别是幼年慢性粒单核细胞白血病 (JMML)检出率较高。Chen 等[ 16 ] 分析了17 例儿 童MDS,只有1 例检测到PTPN11 的突变,此患 儿为RAEB-t,染色体核型为45,XY,-7。
CBL(casitas B-lineage lymphoma) 原癌基因 的体细胞突变近年来在髓系肿瘤中的研究报道很 值得关注,可以证明CBL 突变会造成异常的酪氨 酸激酶信号,从而导致RAS 信号通路的激活[ 17 ]。 Shiba 等[ 18 ] 检测了24 例儿童MDS 样本,均未检测 到CBL 突变。
立早反应基因(immediate early response 3, IER3)位于6p21,编码一种糖蛋白,在调控凋亡 和细胞周期进程中发挥重要作用[ 19 ]。近来的研究 表明,IER3 基因突变在成人MDS 患者中常有发生, 且不局限于伴有6 号染色体异常的患者,IER3 的 低表达水平与预后不良相关。de Vries 等[ 20 ] 检测了 58 例儿童MDS 患儿(其中17 例伴有6 号染色体 异常)中IER3 表达的情况。结果显示,IER3 表 达水平在MDS 患儿中较正常对照低,而不论是否 有6 号染色体异常。这种低表达水平与预后不良 相关。他们还证实,IER3 下调并不是由IER3 启 动子区的超甲基化所引起的。 3 表观遗传学改变
DNA 甲基化是最早发现的表观遗传修饰方式 之一,由DNA 甲基转移酶(DNA methytrans-ferase, DNMT)催化,将甲基转移至胞嘧啶的5 位碳原子 上,形成5- 甲基胞嘧啶的化学修饰过程。在成人 MDS 中常可以观察到异常的DNA 甲基化,包括基 因组整体甲基化水平降低、DNA 甲基转移酶活性 增高、特定位点的异常高甲基化等。这被认为是 MDS 发病和进展的重要因素[ 21 ]。
p15INK4B 和p16INK4A 基因是细胞周期依赖 性激酶(CDK)抑制因子家族的成员,控制细胞从 G1 到S 期的进程,它们合成的产物可以启动一个 抑制细胞生长的重要通路。Rodrigues 等[ 22 ] 研究了 47 例儿童原发MDS 患儿p15INK4B 和p16INK4A 蛋白基因的甲基化状态。32%(15/47) 检测到 p15INK4B 基因的异常甲基化,而p16INK4A 的 异常甲基化只有4 例。超甲基化p15INK4B 在 RAEB 和RAEB-t 两种亚型中出现的频率明显增 高,p16INK4A 的异常甲基化也更多地出现在进 展亚型中。有17 例患儿发生了向AML 的演变, p15INK4B 和p16INK4A 的甲基化状态随着疾病 的进展更加显著。这些结果表明,p15INK4B 和 p16INK4A 蛋白基因的甲基化是儿童MDS 进展的 一个表观遗传标记。Hasegawa 等[ 23 ] 检测了9 例儿 童MDS 患儿中p15 和p16 的甲基化状态,78%(7/9) 的患儿具有p15 基因的超甲基化,这与成人患者 中的概率相似,而所有标本均未检测到p16 基因 的超甲基化。他们认为去甲基化药物可能对多数 MDS 患儿有效。
基因启动子区异常甲基化导致不正常的基因 沉默,抑癌基因的转录抑制是由启动子超甲基化 的数量决定的,这与肿瘤的启动和进展有关[ 24 ]。 Vidal 等[ 25 ] 收集了21 例MDS( 均为RAEB 和 RAEB-t)患儿的骨髓,分析了13 个基因的启动子 甲基化状态。在这些样本中发现了2 个甲基化频 率较高的基因:CALCA(85.7%)和CDKN2B(50%)。 结果显示儿童MDS 基因启动子的异常甲基化发生 率似乎与成人MDS 患者类似。Kim 等[ 26 ] 分析了 25 例儿童MDS 中CDKN2B 的甲基化数量。儿童 MDS 相对正常儿童具有较高的CDKN2B 甲基化水 平,相对于成人患者则较低。而在所有儿童MDS 病例中,原始细胞数>5% 以及染色体核型异常的 患儿较其他患儿显示出更高的甲基化水平。这表 明CDKN2B 甲基化与儿童MDS 的发病与进展有关, 儿童与成人患者间甲基化水平的差异可能是由于 随着年龄增长,肿瘤抑癌基因的甲基化引起的。 4 新的分子学因素
正常细胞含有不同数量的线粒体,某些线粒 体可能同时携带野生型和突变型的线粒体DNA (mitochondrial DNA,mtDNA)。在大约50%~60% 的成人MDS 患者中可以观察到体细胞线粒体DNA 突变,出现的频率随年龄增长而增加,且在进展 型的MDS 患者中频率较高[ 27, 28 ]。de Vries 等[ 29 ] 研 究了mtDNA 突变是否会在一个儿童MDS 的队列 中出现及出现的频率。19 例患儿中有13 例(68%) 检测到了非正常的mtDNA 突变,7 种突变被认为 具有潜在致病性,且线粒体突变状态并不限于某 一个亚型。值得注意的是,这个概率比成人MDS 要高,而mtDNA 突变本身应当是随年龄老化而增 加的。研究者假定线粒体DNA 突变可能会引起造 血干细胞遭受二次打击时的脆性增加,从而促进 了儿童MDS 的发生。这是否是一个诱发儿童肿瘤 的通常途径还需要进一步证实。
最近的研究显示核microRNA 转位进入线粒体 基因组,调节线粒体蛋白合成,进而发挥氧化磷 酸化功能[ 30 ]。microRNA 是人类造血干细胞的调节 者,它们的控制解除会导致血液肿瘤的发生,在 儿童AML 中不同细胞遗传学亚型之间的microRNA 表达量不同,说明其在白血病的生物学中发挥作 用,同样也有证据表明microRNA 的控制解除与 MDS 的发病相关[ 31, 32 ]。
剪接体(spliceosome)是由RNA 与蛋白质剪 接体次单位所组成的复合物,用来将mRNA 序列 中转录自DNA 模板的内含子移除并将剩余外显 子连接起来,在真核生物mRNA 的成熟过程中发 挥了重要作用,剪接体基因突变常常发生在成人 MDS[ 33, 34 ]。Hirabayashi 等[ 35 ] 对187 例原发MDS 和 68 例继发MDS 进行了SF3B1、 U2AF35 和SRSF2 的重新测序,只有1 例RCC 检测到错义突变。剪 接基因突变在儿童MDS 中是罕见的,不可能是儿 童MDS 发病的启动突变。 5 总结与展望
MDS 在儿童中是一种少见病,造血干细胞移 植是唯一可以治愈此病的治疗手段[ 36, 37 ]。到目前 为止,关于儿童MDS 发病机制的研究很少。通过 本文可以很清楚地看到,MDS 在儿童与成人中生 物学特点有所不同。这些研究结果表明,与成人 MDS 甚至儿童AML 相比,现在已知的髓系肿瘤突 变在儿童MDS 中的发生率较低,尤其是影响组蛋 白修饰和DNA 甲基化的突变在儿童MDS 发病中 的作用较小,说明这些类型的突变可能不是儿童 MDS 发生的启动因素。近年来,大量证据证实异 常甲基化在成人MDS 发生和发展过程中发挥重要 的作用,地西他滨等DNA 甲基化转移酶抑制剂用 于治疗成人MDS 也取得了一定的成效[ 38, 39, 40 ]。儿童 MDS 中异常甲基化的发生概率与成人相似,这一 机制可能影响了儿童MDS 的发生,部分研究者认 为MDS 患儿可能会从去甲基化治疗中受益。另外, 一些其他的研究角度如mtDNA 突变与儿童MDS 发 病的联系也值得注意。总之,现在对于儿童MDS 的分子发病机制还知之甚少,需要更多更加深入 及多角度的研究来探索儿童MDS 的疾病本质,从 而指导有效的临床诊治。
