营养不良在慢性肝病患儿中比较普遍,肝移植治疗技术的不断提高使得终末期肝病患儿获得新生,但等待肝移植的慢性肝病患儿中60%~80%的儿童存在不同程度的营养不良,而营养不良的存在也会对肝功能产生不良影响。甚至影响移植的进行[1]。因此,近年来慢性肝病儿童的营养问题越来越受到关注。早期识别营养不良并及早干预有助于改善肝病预后。然而,目前众多的营养评估方法都不能全面、真实反映慢性肝病患儿的营养状况,国内外也缺乏儿童慢性肝病营养状况评估的金标准。本文综述儿童慢性肝病的营养特点及慢性肝病患儿营养状况的评估方法,以期对慢性肝病患儿营养不良的早期识别及干预起到一定的指导作用。
1 儿童慢性肝病的营养特点肝脏在人体内充当着复杂的角色,具有众多生命攸关的功能,包括分泌胆汁、代谢功能(糖类、脂质、蛋白质、维生素、激素)、凝血功能、解毒作用、免疫作用。因此肝脏在调节营养状态和能量平衡中起到重要作用,肝脏发生病变时会对患儿的营养状况产生不良的影响。引起儿童慢性肝病的病因很多,非酒精性脂肪肝、病毒性肝炎、胆道闭锁性肝炎、肝硬化等均可引起。慢性肝病患儿中出现的消化道症状(恶心、呕吐、厌食、腹胀)、锌元素的缺乏、腹水等可使营养摄入减少,同时胆汁分泌的减少、门脉高压引起的消化道黏膜肿胀可致消化吸收不良,而慢性肝病患儿的能量高代谢状态则进一步加剧营养不良。总之,肝病患儿营养摄入的减少、消化吸收不良、高代谢状态是导致肝病患儿营养不良的主要原因,也是慢性肝病患儿的主要营养特点。
2 营养不良对肝功能和慢性肝病患儿预后的影响营养不良对肝功能损害的认识可追溯到上世纪40年代,当时已经报道非发达国家严重蛋白质-热卡营养不良儿童中存在肝脂肪堆积[2]。近来有研究指出营养不良增加了儿童肝移植术后的死亡率和胆汁淤积性肝病的发生[3]。肝硬化患者的营养不良与并发症(肝性脑病、肝肾综合征、静脉曲张出血伴明显肝性脑病、自发性细菌性腹膜炎和肝肾综合征伴明显的肝性脑病等)及死亡率的增加有关[4-5]。相关研究发现,改善等待肝移植的终末期肝病患儿的营养状况可以改善预后[6]。近期的Meta分析显示无论是经口喂养还是肠道喂养都能改善肝病患者的预后[7]。慢性肝病患儿营养不良的存在不利于疾病恢复,早期正确识别是否合并营养不良是及早干预的前提。因此,正确评估慢性肝病患儿的营养状况很有必要。
3 儿童肝病营养评估 3.1 人体测量学指标人体测量学指标主要包括体重、Kaup指数或体质指数、三头肌皮褶厚度、上臂围等。这些指标在营养状况评估中占有重要地位[8],以快速、方便、经济的优势被广泛应用于营养状况评价。
体重和体质指数(body mass index, BMI)是评价营养不良最常用的人体测量学指标。BMI在儿童称Kaup指数,即体重(kg)/[身高(m)]2,其临床意义与成人BMI是相同的。Kaup指数评定标准: > 22为肥胖,22~ < 19为优良,19~ < 15为正常,15~ < 13为消瘦,13~ < 10为营养不良。但体重与Kaup指数受水肿及腹水的影响较大,往往会低估合并有腹水、水肿患儿的营养不良程度。
三头肌皮褶厚度(triceps skinfold, TSF)和上臂围(mid-upper arm circumference, AMC)已经被广泛应用于全身肌肉量的评估。而皮下脂肪和肌肉组织的减少是营养不良的重要表现[9-10],因此TSF和AMC能够用于营养不良的评估。AMC、TSF是判断肝硬化患者肌肉组织量减少的良好指标,而肝硬化患者肌肉组织量的减少与死亡率增加、生活质量下降有关[11]。国外一项研究显示,皮褶厚度是与双能量X线吸收评价法相当的评价方法[12]。Sokol等[13]发现三头肌皮褶厚度在评价急性营养不良方面较体重/身高更加敏感。有学者认为,上臂肌围和三头肌皮褶厚度对预测肝硬化患者生存率可能有帮助,并且上臂肌围的预测能力比三头肌皮褶厚度高[14]。但需要注意的是,慢性肝病患者肌肉萎缩的常见部位可能不是上臂,尤其是那些没有丧失活动能力的患者[15]。
此外,拇内收肌厚度作为一项评价肌肉组织质量变化的指标,受水肿、腹水的影响较小,被广泛应用于手术、肾衰、癌症患者的营养状况评价[16-18],但尚未见应用于慢性肝病的相关报道,其价值尚有待进一步研究。
3.2 生化指标血浆蛋白(白蛋白、前白蛋白、转铁蛋白)水平、肌酐-身高指数、淋巴细胞总数等是反映机体营养状况的客观指标。
血浆蛋白水平包括白蛋白、前白蛋白、转铁蛋白。白蛋白半衰期为14~20 d,是反映机体慢性营养不良的指标。前白蛋白半衰期约2 d,可反映机体近期的营养变化[19];而且半衰期短,相对白蛋白其敏感性更高。但白蛋白、前白蛋白都易受肝功能、急性炎症等因素的影响,影响评估准确性。转铁蛋白半衰期较短,Cho等[20]认为它可以作为慢性乙肝患者肝纤维化的生物学指标。但易受血清铁水平的影响,因此也不是评价营养状况的良好指标。
肌酐-身高指数(creatinine height index, CHI)是指受试者24 h尿肌酐排出量(mg)与同身高标准体重24 h尿肌酐排出量(mg)的比值,可作为评估瘦体组织的可靠指标。有研究表明CHI能够很好地反映慢性肝病患者的肌肉量和蛋白质水平[21]。CHI评价标准[22]:90%~ < 80%为轻度营养不良,80%~60%为中度营养不良, < 60%为重度营养不良。其与肌肉组织总量、体表面积、体重密切相关,且不受水肿、腹水的影响,是终末期肝病模型评分的重要参数之一。但CHI易受肾功能的影响,肾功能不全时不宜应用此法。
淋巴细胞总数(total lymphocytes count, TLC) < 1 500/mm3可能存在营养不良, < 900/mm3可能为严重营养不良。然而TLC易受感染因素的影响,慢性肝病患儿大多合并感染,因此TLC对于此类患者应用价值不大。
此外,生长激素释放肽可能也与慢性肝病患者营养不良有密切关系。生长激素释放肽血症可能是营养不良代偿的结果,可作为早期识别和评价慢性肝病患者营养不良严重程度的指标[23]。
3.3 维生素和微量元素维生素和微量元素的缺乏在慢性肝病患儿中并不少见,特别是终末期肝病患儿。摄入的减少、胆汁产生的减少与分泌障碍,以及载体蛋白合成的减少是慢性肝病患儿维生素尤其是脂溶性维生素缺乏的主要原因。有研究证明脂溶性维生素缺乏与肝病的严重程度有关[24]。此外,肝硬化患者锌缺乏的发生率很高,并且缺乏程度与病情正相关[25-26]。有研究认为监测血清锌的水平有助于早期早期发现肝性脑病[27]。而且锌元素的缺乏也会导致食欲下降从而加重营养不良。因此,对于肝病患儿进行维生素和微量元素的评估很有必要。
3.4 几种营养评价工具营养风险筛查(nutrition risk screening, NRS2002)是欧洲肠外肠内营养学会(ESPEN)在2002年制定并推荐使用的住院患者营养风险筛查方法[28]。NRS2002总评分包括疾病严重程度评分、营养受损评分、年龄评分3个部分。NRS2002在灵敏度方面相对于其他营养筛查工具具有绝对的优势[29],也是中国ESPEN推荐的住院患者营养状况评估的首选工具。NRS2002虽然灵敏度相对较高,但其特异度较低,容易出现假阳性,而且对于合并有诸多并发症的终末期肝病患者来说尚有限制,主要适用于肝病早期及轻症患者[30]。NRS2002虽然由于易受水肿、腹水的影响导致其在肝病患者中的应用受限,但可用于评估营养状态动态变化,判断营养干预的疗效。
主观全面评定(subjective global nutritional assessment, SGA)也是欧洲ESPEN学会推荐的营养评价工具[28],包括近期体重下降程度、饮食变化、消化道症状、生理功能状态、皮脂和肌肉的消耗程度、体液平衡情况等。SGA以详细病史、体格检查为基础,但缺乏实验室指标,其敏感度较低,可能会低估慢性肝病(不包括进展性肝硬化)患者的营养状况[31-32]。为了避免此缺点,可以先使用NRS2002进行初筛,然后应用SGA作进一步评估。另外SGA虽然能预测肝移植术后结局,但是不能预测肝硬化的并发症等情况[33]。SGA的评估大多依靠主观判断,结合其他客观评价指标可以提高SGA评估的准确性。因此,英国皇家自由医院提出了一种改良主观全面评定(royal free hospital-subjective global nutritional assessment, RFH-SGA),在SGA的基础上加入质量指数、上臂围、膳食变化情况三项指标[34]。可以可靠地预测终末期肝病患者肝移植的预后[35]。但是RFH-SGA比较耗时,因此皇家自由医院又推出了可以快速评估营养状况的营养优先工具。营养优先工具识别肝硬化高危患者营养不良的敏感度是100%,特异度73%[36]。而且Borhofen等[37]认为营养优先工具是肝硬化患者肝功能恶化和生存率评估的独立预测因子。
肝病病人营养评价方法(nutritional assessment for liver disease, NALD)[38]是于卫红等综合TSF、AMC、ALB和活动能力4项指标制定的一种用于慢性肝病患者的营养评价工具,它兼顾主、客观,能反映出患者营养状态和肝病的严重程度,更适合肝病病人。但有效性有待进一步验证。
儿科营养不良评估筛查工具(screening tool for the assessment of malnutrition in pediatrics, STAMP)是2008年McCarthy等提出的一种儿科营养不良筛查工具,包括身高体重的变化、疾病状况及膳食情况3部分,据最后综合评分判断为低度营养风险、中度营养风险、高度营养风险[39]。欧洲营养协会推荐使用STAMP对住院患儿进行营养风险筛查[40],但有研究表明其敏感性不高[41]。
4 总结与展望营养不良在儿童慢性肝病中很普遍,并可能对预后产生不良影响。但由于肝病患儿营养评估的复杂性,以及临床医生在营养不良对疾病预后的影响方面估计不足,导致目前慢性肝病营养不良的诊断率低。虽然营养状况的评估方法有很多,但尚缺乏专门针对肝病患儿营养状况的评估工具。TSF、AMC以及RFH-SGA、营养优先工具、NALD在评估慢性肝病患儿营养状况的敏感度和特异度较好,可优先选择。当患儿出现腹水、水肿时不建议以体重、BMI以及NRS2002等作为优选评估方法。对于慢性肝病患儿,不建议以白蛋白、前白蛋白、转铁蛋白等生化指标评估营养状况,但生长激素释放肽似乎可早期识别和评价营养不良严重程度。需要注意的是,以上方法各有优缺点,都不能完全准确、有效地评估肝病患儿的营养状况。因此,慢性肝病患儿营养状况的评估应该以详细的病史、体格检查和必要的辅助检查为基础,结合各营养评估方法的适用范围和优缺点进行工具选择,同时在临床中积极寻找新的有效的营养筛查工具,以早期发现慢性肝病患儿营养不良的情况并及时干预,从而改善疾病预后。
