微生物-免疫-代谢轴在儿童肥胖相关性哮喘中的研究进展
Research progress on the microbiota-immune-metabolism axis in childhood obesity-related asthma
儿童哮喘及肥胖的患病率均在逐年攀升,肥胖相关哮喘成为独特的哮喘临床表型,儿童肥胖不仅是哮喘的诱发因素,更是驱动疾病进展及影响疗效的关键病理生理环节。肠道菌群及其代谢产物的组成在肥胖状态下发生改变,通过肠-肺轴激活肺部免疫,而肥胖引起的全身性慢性炎症及代谢紊乱与肺部免疫失衡、气道重塑及气道高反应性等密切相关,共同构成微生物-免疫-代谢轴。该文综述了肥胖通过微生物-免疫-代谢轴影响儿童哮喘的潜在机制,旨在探索和深化对于肥胖影响儿童哮喘机制的认识,为临床干预及转化提供研究思路及靶点。
Childhood asthma and obesity are rising in prevalence, and obesity-related asthma has emerged as a distinct clinical phenotype. In children, obesity is not only a trigger for asthma but also a key pathophysiological driver of disease progression and a modifier of treatment response. In obesity, shifts in the composition of the gut microbiota and its metabolites can activate pulmonary immunity through the gut-lung axis, while obesity-induced chronic systemic inflammation and metabolic dysregulation are closely linked to pulmonary immune imbalance, airway remodeling, and airway hyperresponsiveness. Together, these processes constitute the microbiota-immune-metabolism axis. This review summarizes potential mechanisms by which obesity influences childhood asthma via this axis and aims to deepen mechanistic understanding and provide research directions and targets for clinical intervention and translation.
