脑白质个体化功能网络在儿童青少年注意缺陷多动障碍中的应用进展
Advances in the application of individualized white-matter functional networks in children and adolescents with attention-deficit/hyperactivity disorder
脑白质个体化功能网络为解析儿童青少年注意缺陷多动障碍的个体差异与神经机制提供了新范式。该文综述脑白质个体化功能网络构建方法(聚类分析、独立成分分析等)及研究进展。注意缺陷多动障碍的群体水平研究发现,患儿默认模式网络及感觉运动网络相关的脑白质功能连接增强,注意网络相关的脑白质功能连接减弱,且这些网络连接异常与核心症状及执行功能损害相关。但群体水平分析会掩盖个体间的网络异质性,忽视灰质-白质功能耦合,阻碍临床转化。脑白质个体化功能网络有望成为儿童青少年注意缺陷多动障碍早期筛查与精准干预的潜在神经影像学标志物,但仍面临方法学标准不一、儿童数据获取困难等挑战。
Individualized white-matter functional networks provide a new paradigm for parsing individual differences and neural mechanisms in attention-deficit/hyperactivity disorder (ADHD) among children and adolescents. This review summarizes construction approaches for such networks, including clustering analysis and independent component analysis, and synthesizes current progress. Group-level studies in children with ADHD have shown increased white-matter functional connectivity in the default mode and sensorimotor networks, but decreased white-matter functional connectivity in attention-related networks; these connectivity abnormalities are associated with core symptoms and impairments in executive function. However, group analyses can obscure inter-individual network heterogeneity and overlook gray-white matter functional coupling, limiting clinical translation. Individualized white-matter functional networks hold promise as potential neuroimaging markers for early screening and precision intervention in ADHD, yet challenges persist, including methodological inconsistency and the difficulty of acquiring pediatric data.
